Services to Prevent and Reduce Isolation, Vulnerability and/or Loneliness in Older People

REDUCING HOSPITAL RE-ADMISSIONS FOR OLDER PEOPLE Happy to Help is working with local services and hospitals to offer a subsidised 6 week home support packages to isolated older and disabled residents returning home from hospital. The programme will complement and enhance personal care packages that form care plans for patients and will support them to manage the home, shop, collect pensions and pay bills while they recover at home. This reduces the emotional burden of not being able to manage the home, prevents isolation and loneliness whilst reducing the proven increased risks of readmission into hospital without additional support. GATESHEAD MEMORY BOOK BLOG – Teams and Bensham Community Care received funding from Awards for All in the Summer to work with Equal Arts to create a memory book about Gateshead. This has far reaching benefits for those taking part, particular for those experiencing memory loss. Richard Bliss, the artist working with our day club members, has helped us to create blog, to share stories from our members in their own words. TRIPS AND ACTIVITIES FOR OLDER PEOPLE – Thanks to funding for Comic Relief, we are able to offer many more trips and activities for local older people. We’re off to the pantomime soon, we’ve been to Heighley Gate Garden Centre and next year we’re planning to go to Beamish and other areas of interest. We also take our members on regular, smaller trips to supermarkets and cafes, or a drive to the Angel of the North for an ice cream in the summer. We are also introducing themed film days, tea dances and various arts and music therapies and memory projects on top of our regular time table of activities.
